Use the form below to select your CBCS clothing items.
Budget limit is $500 per person – please keep track of the total for yourself.

• Please scan over each item and  change the request from “No” to “Yes” if you would like that item.
• Select the size, color and quantity where applicable.
• If you would like more than 1 color of the same item, then you need to fill out a second form.
• Please fill out all of the info at the bottom of the form completely.  Please allow 3-4 weeks for delivery. Embroidery items may come in a separate shipment and take a little longer.
